How do I become a patient?

If your insurance is PPO, you may make an appointment immediately.  If your insurance is HMO, you may call your insurance to change to or add Dr. Lim as your child's provider.  We prefer SCCIPA (Santa Clara County Independent Physician Association) as the medical group.  

If your child is a new patient, you may print the 4 registration forms on adobe files from the Home page under NEW PATIENT REGISTRATION/DOCUMENTS.  Please print and fill the forms prior to your first visit. This will save you some time or you may simply fill the forms when you come in for your child's first visit. Thank you.


Why should I choose Dr. Lim's office over other offices?

What languages does the office speak?

Our medical staff speaks Mandarian, Cantonese, and Vietnamese.  Dr. Lim speaks some Cantonese and Spanish.        


Meet the staff:


Sam Yin, MA (Medical Assistant) graduated from Boston Reed College and has a BA degree in Linguistics and Literature from the University of Social Science and Humanities - Vietnam. She is fluent in Vietnamese, Cantonese, and Mandarian.

Vivian Dang, MA (Medical Assistant) graduated from Silicon Valley College and has an Associate Degree in Business Administration from Evergreen Valley College. She is fluent in Vietnamese.




Can I visit the office or meet Dr. Lim before making an appointment?

Yes, we provide free pre-natal consultation and also pediatric consultations so you can see our office and meet Dr. Lim before deciding on our practice.
